Progress Report
The progress report is your roadmap to training any dog as a service dog from start to finish. The behaviors are divided into 3 levels: 1. Foundation, 2. Intermediate, 3. Specialized. Train the behaviors in the order listed.
Check the cells as you start a behavior, then each time you add criteria.
Behaviors trained
This is the list of the most common behaviors that you will need and the vocabulary that is most common in the service dog industry. Most behaviors are important for public access and obedience. In most cases, you will need 4-7 disability-specific behaviors.
Level 1 Assessment
Once you have trained all the level 1 behaviors, use this assessment to test the dog and make sure they are ready to move on to the next level.
Section B of the assessment will test the dog’s temperament as a service dog in training.
Level 2 Assessment
Once you have trained all the level 2 behaviors, use this assessment to test the dog and make sure they are ready to move on to the next level.

Weekly training log
This form is an example of a weekly log to keep track of the dog’s progress. Log each training session, in the first column, write the behavior you have worked on. The 4 next columns (initials) are to initial every time you do a training session. The last column is to write notes about the session.
